Here’s a question most EHS leaders don’t ask out loud: How do I actually know where we stand?
Not in terms of incident rate or audit score — but in terms of how mature the underlying system is. How consistent it is. How proactive it actually runs day to day, not just when something goes wrong.
Most programs feel more advanced than they are because the gaps aren’t visible. Incidents get reported. Training gets assigned. Checklists get submitted. But the data isn’t connecting, the coverage isn’t real-time, and the processes that look consistent on paper vary significantly by site.

Gap 1: Incident Data That Doesn’t Connect

A near-miss from three months ago and a recordable incident from last week share a root cause. One involved a forklift operating too fast in a congested area. What they often can’t tell you — in real time, across every role, every site, every contractor — is what’s expiring, what was missed, and what the current coverage rate actually is for any given job function or location.

The team scrambles to pull documentation. Folders are searched. Old spreadsheets are updated. Records that should be centralized and current are assembled from three different sources.
This pattern isn’t just operationally inefficient. It tells auditors, insurers, and operational leadership something about the underlying maturity of the program: that readiness is a state you enter, not a state you maintain.
Programs at the Developing or At-Risk maturity level often have the components of audit readiness — records exist, documentation has been maintained, compliance milestones have been tracked. What they lack is the infrastructure to surface it instantly. The difference between producing documentation in 20 minutes and producing it in two days isn’t a records-management issue. It’s a systems issue.
Strong programs aren’t audit-ready because they prepare better. They’re audit-ready because their operational data is the audit data. Compliance calendar deadlines are tracked in the same system as corrective actions and training completions. When a review arrives, the documentation is already organized — because the program runs on it daily.
