Supporting our residents and their families through end of life discussions and decisions is difficult for many of us. During this empowering presentation which builds on the Advance Directives class, participants will explore Advance Care Planning including the vital components of life care planning and quality of life into one’s plan for end of life care. Practical guidance and tools will be provided to help facilitate Advance Care Planning conversations with residents and their families, as well as discussion of barriers and solutions. Length: 60 minutes 1 CEU. Approval codes will be avail on the course certificate upon completion of the course.
